We want your first appointment with us to be as efficient as it can possibly be. Please click here to read our patient information guide and complete our Information and Medical Forms in advance.

Please read below for some important information relating to your appointment with us. This will help the appointment process go as smoothly as it can.

Please read our Patient Information and Medical Information forms (below) carefully. Once you have read and understood them, please fill them out, sign them and bring them with you to your appointment.

If you forget to bring your forms, our reception staff can provide you with the forms when you come to the clinic for your appointment. However, you will need to allow at least five minutes prior to the start of your appointment to have enough time to complete them.

Reading and signing our Patient Information and Medical Information forms is compulsory, and unfortunately we will not be able to see you at any of our clinics if you haven’t done so.

What to wear

For lower body problems please bring a pair of shorts with you to get changed into, for shoulder/​back problems a vest top with narrow straps is useful.

If you are seeing a Podiatrist, please make sure to bring your relevant footwear to the consultation. It is advisable to bring both your sports shoes and everyday/​work shoes.

For all appointments, please bring any scans or reports from previous consultations with you. If you haven’t already provided us with this information we also require your GP’s details (name and address) and any other medical practitioner/​referrer who needs to be kept up to date with your progress.

Insurance

Our Clinicians are recognised by all major health insurers. We are able to bill directly to bill AXA Health, AXA Global Health, BUPA, BUPA Global, CIGNA, CIGNA International, AVIVA, WPA, Healix and Health Partners Europe. For all other insurers, we ask that you make payment at the end of each appointment, and we will provide you with the necessary receipts to reclaim from your insurers. It is important that you contact your insurer before you begin treatment to ensure you are covered.

Please take some time to check your health insurance policy so that you know what treatments are covered.

Cancellations

In order to respect our practitioner’s time and other patients, we operate a 24hr cancellation policy. For appointments with our Strength and Conditioning team, the cancellation notice period is 48hrs. If you need to change your appointment please contact reception, any cancellations under the given time frames or non attendance will be charged the full self-paying consultation rate.

When the clinics are closed an answering machine is available to leave messages and we will get back to you asap regarding enquiries, bookings and cancellations.

Reminder text messages and / or emails are sent as a courtesy, however it is your responsibility to ensure that you keep a record of all your future appointments.
