Terms & Conditions: LiveWell Health Assessments

1. The Promoter

The promoter is Pure Sports Medicine Limited, whose registered office is at Point West, 116 Cromwell Road, Kensington, London, SW7 4XR. 

2. Eligibility

• Entrants must be 18 years of age or over.
• Entrants must provide a valid first name, surname, and email address to enter, via the prize draw online entry form.
• Employees of Pure Sports Medicine or their immediate families are excluded at the promoter’s discretion.

3. How to Enter

• Entry requirements will be stated in the promotional materials and online form.
• Only one entry per person is permitted unless otherwise stated.
• Incomplete or illegible entries will not be accepted.
• Closing date for entry is Friday, 27th February 2026 at 5pm. After this date no further entries to the prize draw will be permitted. 

4. Prize Details

There are three first prizes available. Each prize consists of one (1) LiveWell Health Assessment to the retail value of of £300 delivered by Pure Sports Medicine. All unsuccessful entrants will receive a 20% discount off a LiveWell Health Assessment for all unsuccessful applicants. Limit of one prize per winner.

• The LiveWell Health Assessment includes the evidence‑based testing and consultation detailed on the service page at puresportsmed.com.
• The prize must be claimed and booked by 5pm on Friday, 13th March 2026.
• The prize is non-transferable, has no cash alternative, and cannot be substituted for another service.

5. Winner Selection & Notification

• Winners will be selected at random from all valid entries.
• Winners will be notified via email by 5pm on Monday, 2nd March 2026.
• Winners must respond by 5pm on Friday, 13th March to accept the prize and arrange booking. Failure to respond may result in the prize being offered to another entrant.

6. Conditions of the Prize

• Appointments are subject to clinician and location availability.
• Pure Sports Medicine reserves the right to substitute the prize with another of equal value if necessary.
• Pure Sports Medicine reserves the right to cancel or amend the prize draw and these terms without notice in the event of circumstances outside its control (e.g., regulatory changes, operational disruption).
• All winners agree to the use of their name and image in any publicity material, as well as their entry. This includes featuring in content created, filmed and recorded by Pure Sports Medicine for promotional purposes in the form of a patient case study. This is an obligation of all winners of the prize draw. Any personal data relating to winners, or any other entrants, will be used solely in accordance with current UK data protection legislation and will not be disclosed to a third party without the entrant’s prior consent.

7. Data Protection & Privacy

• Personal data collected for prize draw entry will be processed in accordance with the Pure Sports Medicine GDPR Privacy Policy, which explicitly includes processing for prize draws. [PSM_GDPR_P…acy Policy | PDF]
• Data will be used solely for the administration of the competition unless the entrant provides marketing consent.
• This promotion is in no way sponsored, endorsed, or administered by, or associated with any other organisation or business. You are providing your information to Pure Sports Medicine and not to any other party. The information provided will be used in conjunction with the following Privacy Policy.

8. Limitation of Liability

• Pure Sports Medicine accepts no responsibility for entries not successfully completed, technical failures, or any other fault outside its control.
• Pure Sports Medicine provides no warranty beyond what is standard for the LiveWell service.

9. Acceptance of Terms

By entering the competition, entrants confirm that they agree to be bound by these Terms & Conditions.
